Registration
Fellows Institute Registration—Saturday, Nov. 12, 2005
Includes: Presentation materials, breakfast, lunch with roundtable discussions, and a separate CE Certificate.
Full Three-Day Conference Registration
Includes: Access to all sessions and exhibit hall, all conference meals, access to the Saturday Welcome Reception in the Exhibit Hall, access to the Barbara Stein Memorial Lecture/Reception and Silent Auction, and one copy of the conference proceedings book.
One-Day Conference Registration
Includes: Access to the day's sessions and exhibit hall, conference meals for that day, and one copy of the conference proceedings book. Added benefit the one-day registration will include the Saturday Welcome Reception in the Exhibit Hall or access to the Barbara Stein Memorial Lecture/Reception and Student Auction.
Guest Registration
Guest registration is limited to the Saturday Welcome Reception and Barbara Stein Memorial Lecture/Reception. Guest Registration may not be used to attend educational sessions.
Dry Lab Registration
Registration for a Dry Lab is separate from the conference and will be confirmed on a “first come, first served” basis, with preference given to Fellow Members and those registering for the full conference. Class size is limited to 24 registrants per lab and includes: a separate CE Certificate, presentation materials, and afternoon coffee break.

Conference Proceedings
The AAFP Conference Proceedings Book has grown and now includes an increased number of PowerPoint Presentations and handouts. If you attended the 2005 Winter Conference in Puerto Rico, you would have received the new, improved book which allowed attendees to take notes right next to the slide that the speaker was referring to.
The book contains numbered tabs so it is easy to locate the sessions, take notes and refer back to your notes when you return to your practice. Due to the increased size and cost of proceedings book we will not be providing a CD as part of your registration fee. The Conference CD may be purchased separately during or after the conference.

The hotel is situated on the corner of State Street and Wacker Drive, between the Loop and the Magnificent Mile. This convenient address places you in the heart of downtown.
All room reservations must be made before Friday, October 21, 2005. After this date there will be no guarantee of available rooms. All reservations must be accompanied by a first night room deposit, or guaranteed with a major credit card. AAFP has contracted a block of sleeping rooms at the following group rate. Hotel room rates are subject to applicable state and local taxes, currently 14.9%.

The AAFP encourages participants in the AAFP event to book their overnight accommodations at the event headquarters hotel, The Chicago Renaissance. The AAFP makes every effort to secure the best possible rate for overnight accommodations. This rate for overnight accommodations is part of a negotiated event package that incorporates meeting room rental, meals and breaks, as well as other services associated with conducting the AAFP event. If the AAFP does not achieve the minimum number of overnight accommodations required in the overall event package, it affects the price of other services associated with conducting the AAFP event. This, in turn, impacts registration fees for future programs.
