Evidence for a defined syndrome is summarised with the conclusion that this is overwhelming. Causation is explored and a practical way of identifying and managing the syndrome in primary care is detailed; this includes the necessary lifestyle changes, the appropriate use of drug therapy and the selective use of surgery.
