Thank you for your letter for this academic question. I have read this article and many similar documents. There are two kinds of methods through the out sheath of the resectoscope, most of the methods reported in the literature are not using cap sealing, in fact, to put together a sealing cap is often inappropriate. This method is interesting and similar to my invention, the letter's method that the out sheath of the resectoscope allic irrigator connection sealing cap. However, there are many methods for treating bladder calculi, including similar methods as shown in the figure, I have tried similar methods and these are not as good as my method.

EMS (Ultrasonic pneumatic ballistic lithotripsy system) is a good method because there are two ways to break the stones and with adsorption function. However, all the treatment methods should be based on the equipment available at the medical institutions, not all hospitals have EMS. Many hospitals in China and in other countries in the world do not have EMS because it is expensive.

The figure shows clearly that flow from the top of the entrance into the outflow from the following export cannot continue to reach the bladder irrigation, and export is very small so that the actual operation of traffic is not large enough, the field of vision is still not good, and the stones are not so easily removed.

We have used the out sheath of the resectoscope many times in the clinic, and the results were disappointing. So we have modified our approach, hoping to improve the operation efficiency. In fact, in addition to animal experiments, we have also tried this method in the clinic and is significantly better than the external sheath of the resectoscope, and we got a lot of peer recognition. Soon, this method will be seen in clinical research and surgical video.

Personally, the out sheath approach is less effective than ours. If there is a chance, you can compare these two methods.
