• Clarification on Page 670, in Table 9: For TS1, First combination, the Requirements for diagnosis should read, “Thyrotoxicosis and at least one CNS manifestation and one of the following: fever, tachycardia, CHF, or GI/hepatic manifestations”.

• Correction on Page 671, right column, third paragraph, lines 4–6: The sentence beginning, “Second, the surveys …” should read, “Second, the surveys were performed in Japan only, and the response rates for SURVEY-1 and SURVEY-2 were only 52.5% and 80.8%, respectively.”

• Clarification on Page 673, left column, under Diagnostic criteria for “definite”: The list item preceded by “(i)” should read, “At least one CNS manifestation plus one of the following: fever (≥38°C), tachycardia (≥130 beats/min, see also Note 3), CHF (Note 4), or GI/hepatic manifestations (see also Note 5).”
