Abstract

I read with interest the excellent review by Dr. Yeh of the recent breakthroughs in surgical thyroidology (1). As I was about to finish the reading, I came across Figure 2 comparing the trolley problem (introduced by Philippa Foot) with the ethical dilemma clinicians are facing when dealing with papillary thyroid microcarcinoma (2). Surprisingly I had the same idea several months ago and, actually, I published it not once but twice. My first contribution was on the Internet (in Spanish) on May 10, 2019 (3) and, a few months later, in an editorial (this time in English) in the Spanish journal Endocrinología Diabetes y Nutrición (written in July 2019, available online on September 16, 2019 and published in October 2019) (4).
I think that it is rather unlikely that two people could come up with the same analogy about this particularly trendy topic. While recognizing the trolley problem is not exactly obscure, could Dr. Yeh have nevertheless found inspiration in my articles. Is it me who is off his trolley?
