Abstract

1. Bilateral Morgagni hernia: operative discovery of appendix lying on superior pulmonary vein
Cardiothoracic surgeons based at Little France, Edinburgh describe an exclusive surgical case. Clinical clues are acute abdominal pain and worsening respiratory symptoms in an obese, asthmatic patient. Preoperative chest X-rays, four CT scans and nine references are provided.
2. Acute myocardial infarction in pregnancy following unlicensed use of methylenedioxymethamphetamine (‘ecstasy’)
The agony of ecstasy through released catecholamine-induced platelet aggregation sets the scene for thrombus formation in a pro-coagulant pregnant woman. A happy ending follows percutaneous cardiological intervention and a normal delivery of a live infant.
3. Improvement in symptoms of peripheral arterial disease with sildenafil therapy
A patient with intermittent claudication has erectile dysfunction, for which he is prescribed phosphodiesterase type-5 inhibitor, sildenafil. He now uses sildenafil regularly to complete his weekly shopping trips rather than for its licensed indication. Every little helps.
4. Cozen’s phenomenon: a reminder
This unpredictable and potentially under-diagnosed condition often follows an individual path. Early recognition of children at risk is imperative to help prevent distress at a later stage. Paediatric orthopaedic advice is recommended.
5. Dipyridamole and paracetamol overdose resulting in multi-organ failure
A sobering account of a paracetamol and one of the largest documented dipyridamole overdoses highlights near-fatal sequelae: fluid-refractory hypotension, reduced consciousness, acute kidney injury and cardiac arrest. The patient survives albeit without a section of ischaemic small bowel.
6. Neonatal hip septic arthritis: ultrasound should not influence decision to aspirate
At Royal Hospital for Sick Children, Glasgow, surgeons demonstrate intuitive clinical judgement on an infant presenting with suspected septic arthritis. They discuss the false negative rate associated with ultrasound and the need for arthrotomy and washout rather than serial aspiration.
7. Conservative management of laryngeal dogbite
Originally an abstract of a poster presentation at the British Association of Paediatric Otolaryngologists meeting in September 2010, this detailed case report provides an algorithm for the management of dogbite related injuries illustrated by 13 serial photographs and radiological images.
8. Two isolated giant gastric duplication cysts in thorax in a newborn
A Turkish multi-disciplinary team saves a newborn infant with severe respiratory distress. Two giant and separate enteric duplication cysts were removed from the right thorax day 19 and the patient was discharged 3 days later. This report is a first in the literature. The diagnostic values of radiological evaluation and surgical and pathological management for precise diagnosis are discussed.
