Human performance-related events having safety and operational effectiveness implications at commercial nuclear power plants have been the subject of continued concern. Several recent events have been investigated by the U.S. Nuclear Regulatory Commission (NRC) staff. The purpose of these investigations was to identify the root cause(s) of the human errors involved and to provide guidance to the nuclear industry on how to reduce the probability of occurrence.
