A 71-year-gentleman presented in the morning surgery with central abdominal pain, crampy in character. The pain is associated with nausea, vomiting and diarrhoea. The pain lasted for about 2 hours but seems to have settled. His vital signs and abdominal examination were unremarkable so you reassured him and sent him home. Later in the afternoon his wife asks for a home visit because his pain has recurred and he is deteriorating quickly.
