Systematic assessment of the wound should be undertaken including careful inspection of the skin surface. The labia should be parted and any internal component of the wound should be inspected. It is not usually necessary to attempt speculum examination unless concerned about endometritis. The wound should be swabbed and empirical broad spectrum antibiotics should be commenced. The most commonly isolated organism is group A streptococcus. Occasionally perineal infections may spread very quickly and result in overwhelming sepsis.
