A mother brings her 10-year-old boy to see you in emergency surgery. He has been unwell since this morning with fever, headaches, photophobia and seems to be confused. He is deteriorating very quickly. On examination his temperature is 39.4°C, he is photophobic, his neck is very stiff and you notice an area of non-blanching rash on the left side of his abdomen.
