Answer E. Trial of orlistat
Obesity increases the risk of diabetes and cardiovascular disease. An initial weight loss of 5–10% is suggested for patients with diabetes. Recommendations include at least 45–60 minutes of moderate intensity exercise every day with a low calorie daily diet of 600 calories. A calorie intake of less than this should only be suggested by specialists. If lifestyle measures do not work, then orlistat can be initiated if the body mass index (BMI) is over 28 kg/m2. Bariatric surgery should be considered if the BMI is over 35 kg/m2. Sibutramine has been withdrawn because of associated cardiovascular risks. Saxagliptin can be used to help with diabetic control but does not help with weight reduction.
