Answer E. Very urgent referral to the paediatric team (within 48 hours)
Children with unexplained visible haematuria should be referred very urgently (within 48 hours) to paediatrics to investigate for Wilm’s tumour. Hospital admission is not warranted if the patient is asymptomatic and there are no signs of glomerulonephritis. Urine can be dip tested for protein but ideally a mid-stream specimen of urine should be sent to look for evidence of red blood cell casts. There are no symptoms that suggest a urinary tract infection so antibiotics are not recommended. Routine referral would be appropriate for asymptomatic children with persistent non-visible haematuria.
