Answer C. Kallman’s syndrome
This boy does not have symptoms of puberty consistent with his age. The normal age for puberty in boys is 9–14 years and therefore this presentation would indicate delayed puberty. His low gonadotrophin levels suggest he has hypogonadotrophic hypogonadism. The lack of smell would suggest a diagnosis of Kallman’s syndrome. Constitutional delay of growth and puberty is usually accompanied by a positive family history, malnutrition or chronic disease. Congenital adrenal hyperplasia and Neurofibromatosis type 1 are causes of precocious puberty. Klinefelter syndrome causes hypergonadotrophic hypogonadism and so the luteinising hormone (LH) and follicle secreting hormone (FSH) levels would usually be high in this case.
