Answer E. Trial montelukast 4 mg daily in the evening
If patients remain symptomatic despite the top dose of inhaled corticosteroid (400 microgram), a trial of a leukotriene receptor antagonist should be considered. Long acting beta agonist therapy is not recommended for children under the age of 5 with asthma. Antibiotic therapy is not indicated here. The top dose of inhaled corticosteroids is 400 microgram daily in this age range (as supposed to 800 microgram daily after the age of 5 years). In children under the age of 2 years, rather than trying a leukotriene receptor antagonist, referral to a paediatrician would be appropriate.
