A young mother brings 4-week-old Laura to see you because Laura is constantly crying. Laura is bottle fed and always distressed but noticeably worse after feeds. Laura tends to choke while feeding and mum can hear gurgling sounds in her throat. Laura does not sleep well, cries when laid flat and prefers to be held upright. Laura brings up a small amount of milk after feeding but there is no history of vomiting. She is thriving and steadily gaining weight. Examination is unremarkable.
