Answer D. Urgent referral to the general surgical team
The history and examination in this scenario is suggestive of acute appendicitis. Patients presenting to primary care with suspected appendicitis need an urgent referral to the general surgeons for review and potential admission for inpatient surgery. In some cases when the diagnosis is unclear, appendicitis is not suspected and the patient is systemically well, it is a safe option to counsel the patient about the natural history of the disease, to ‘watch and wait’ and review after 24 hours.
