A 35-year-old lady comes to see you with a ‘typical flare-up of her sinusitis’. She presents with severe frontal headache with some nausea. She describes intermittent fevers over the past few days. She has pain behind her right eye but has had this before with her ‘migraines’.
On examination, her temperature is 38.5°C, her other observations are normal apart from a slightly raised blood pressure of 160/95 mmHg. She has neck stiffness with photophobia but fundoscopy is normal.
Arrange a CT scan of her sinuses Immediate admission to the medical assessment unit Refer to ear, nose and throat (ENT) specialists for ‘recurrent sinusitis’ Treat with amoxicillin and ibuprofen Treat with intranasal decongestants
