The clinical picture is suggestive of premature ovarian failure (POF). Follicle stimulating hormone (FSH) and luteinising hormone (LH) are high in premature ovarian failure owing to lack of negative feedback from the ovaries on the hypothalamus. A gynaecological referral should be made for patients with suspected premature ovarian failure under the age of 40 to confirm their diagnosis. Such patients should be screened for autoimmune disease, monitored and treated for reductions in bone density, with referral for fertility treatment where appropriate. Women less than 30 years of age may require karyotyping to exclude genetic causes.
