A 69-year-old man presents with a 3-month history of lower back pain. This keeps him up at night and can be there in the morning. There is no evidence of sciatica. There are no urinary or bowel symptoms. This pain has got persistently worse and he has been using increasing amounts of paracetamol and codeine without effect. He says he feels constantly tired. He has lost his appetite recently and he thinks he has lost a little weight.
