Answer A. Endometriosis
Endometriosis presents with pelvic pains and deep dyspareunia. Women can also complain of menorrhagia. The diagnosis can be quite delayed and the gold standard diagnostic test is laparoscopy. Fibroids can present with dysmenorrhoea and menorrhagia. Ovarian cancer is less likely given this woman’s age, but symptoms can be quite vague and include bloating, early satiety, continual pain and urinary symptoms. Pelvic inflammatory disease will usually present more acutely with vaginal discharge, intermenstrual or post-coital bleeding and deep dyspareunia.
