Anti-diabetic medications are primarily excreted by the kidneys and so caution needs to be taken when prescribing, especially in elderly patients with chronic kidney disease. Linagliptin is safe to use and preferred in end stage renal disease with an estimated glomerular filtration rate (eGFR) of less than 30. All the other drugs should be avoided at this stage of kidney disease.
