Answer A. Nasal sumatriptan
This boy has already tried simple analgesia and first-line anti-emetic, domperidone. Second-line treatment would include nasal sumatriptan. Oral triptans are not licenced in people under the age of 18 years. Aspirin should be avoided owing to the potential of Reye’s syndrome. Codeine is not indicated for migraines and metoclopramide is licenced in children older than 12 years of age in the non-oral preparations.
