A 56-year-old gentleman with a recent diagnosis of gout comes to see you for a review 4 weeks after starting allopurinol. He is currently on 50 mg daily. He hasn’t had any acute attacks since starting allopurinol. His eGFR is 40 ml/min/1.73m2. His serum urate is 400 micromol/L.
Add febuxostat 80 mg daily Continue the same dose of allopurinol Increase allopurinol dose to 50 mg and 100 mg on alternate days Increase allopurinol dose to 100 mg daily Switch to febuxostat 80 mg daily
