Answer C. Trial of vaginal oestrogen
The definition of recurrent UTI is two or more UTIs occurring within 6 months, or three or more infections in 1 year. Vaginal oestrogen has been proven in multiple systematic reviews to reduce the risk of UTI in postmenopausal women. All postmenopausal women with recurrent UTIs should be offered vaginal oestrogen after discussing the possible benefits of the treatment and possible adverse effects. This option should be considered if lifestyle measures alone have not been enough to prevent urinary symptoms and before antimicrobial treatments are suggested. However, a urology referral in patients with recurrent UTI is indicated to rule out renal or bladder cancer.
