Answer B. Erythrocyte sedimentation rate
People over the age of 60 with back pain (especially if persistent) should be screened for myeloma. This patient already has an anaemia and hypercalcaemia, which also raise suspicion of this diagnosis. A raised erythrocyte sedimentation rate (ESR) can be found in myeloma and would add to the evidence for this diagnosis.
