A 24-year-old lady comes to see you complaining of paroxysmal episodes of shortness of breath. This is usually associated with paraesthesia affecting both hands. She has ended up in the local emergency department a few times. Examination and all her investigations, including blood tests, electrocardiogram (ECG) and a chest X-ray, were normal. The only finding was mild respiratory alkalosis on an arterial blood gas sample.
