Answer D. Topical antibiotics
The clinical picture is suggestive of otitis externa. Drug treatment is by topical antimicrobials; oral antibiotics have poor penetrance of the external ear and should be reserved for those with cellulitis or perichondritis. Sending an ear swab for microscopy and culture is not necessary in most patients but should be considered in those with recurrent or severe infections.
