Abstract
Clinical History:
A 58-year-old man presented with a 1-year history of abdominal distension.
Physical Exam:
Preoperative magnetic resonance imaging demonstrated a 70 × 33 × 40 mm enhancing mass in the left adrenal region, closely related to the left renal vein.
Diagnosis:
Retroperitoneal renal hilar paraganglioma with severe adhesion to the left renal vein.
Intervention:
The patient underwent laparoscopic tumor resection using a left renal vein occlusion technique. Intraoperatively, severe adhesions between the tumor and the left renal vein were identified. The left renal artery and the distal and proximal segments of the left renal vein were sequentially dissected and clamped. The tumor was then carefully separated from the renal vein, allowing complete excision while preserving vascular integrity and avoiding nephrectomy.
Follow-Up/Outcomes:
The operative time was 120 minutes, and estimated blood loss was 50 mL. The postoperative course was uneventful, and the patient was discharged on postoperative day 5. Histopathological examination confirmed retroperitoneal paraganglioma. At 1-year follow-up, no local recurrence or metastasis was detected.
