Sometimes It Really Is the Zebra: A Rare Case of Infantile Giant Cell Fibroblastoma

Plastic surgeons often perform surgical excision of soft tissue lesions and masses on the head and neck. In the pediatric population, these are most commonly benign lesions; however, malignant lesions must always remain on the differential. In this case report, we describe a case of a scalp giant cell fibroblastoma in infancy. Once the diagnosis was confirmed, a multidisciplinary tumor board including pediatric hematology-oncology was consulted for management recommendations. Surgical re-excision with additional 1 cm margins was performed and wound closure was accomplished without any undermining of surrounding skin and ultimately covered with a skin graft. While extremely rare, giant cell fibroblastoma is a locally aggressive soft tissue tumor that plastic surgeons may encounter in clinic practice and requires multi-disciplinary care.
