1. Unnecessary diagnostic investigations in benign acute childhood myositis: a case series report

Dramatic onset of bilateral leg pain with inability to walk associated with coryzal symptoms in previously healthy children without clinical or laboratory evidence of serious pathology does not justify further investigation according to paediatricians from Greece. Authors justify their rationale providing eight references.

2. Vertebral erosion resulting from a ‘non-ruptured’ abdominal aortic aneurysm: case report and literature review

This case report describes an aneurysm sac, the posterior wall of which was absent and abutting the anterior spinal ligaments with erosion of the L4 vertebral body. A literature review identified another 17 documented cases. Computerized tomography and intraoperative images clearly identify the vertebral erosion.

3. Legionella pneumophila serogroup 3 infection: importance of serology

Following the sudden and deadly outbreak of Legionnaire's disease in Scotland in June 2012, this highly relevant case report from Glasgow highlights the importance of considering non-Legionella pneumophila 1 serogroups as causes of community-acquired pneumonia. This paper is critical reading with 17 references and admission chest X-ray.

4. Not hysteria: ovarian teratoma-associated anti- N -methyl- d -aspartate receptor encephalitis

Disabling psychiatric and behavioural symptoms result in treatment under the Adults with Incapacity Act (Scotland) in a previously healthy woman. Encephalitis associated with specific antibodies against neuronal proteins requires immunotherapy to prevent permanent cognitive dysfunction, seizures and ultimately death.

5. Infusion of noradrenaline through the proximal line of a migrated central venous catheter

Despite central line safety bundles to prevent line infections, little is done to monitor their migration. Although this is an uncommon complication, the consequences are serious and potentially fatal. Anaesthetists from the Victoria Infirmary, Glasgow, emphasize the importance of appropriate training in central line insertion to ensure correct placement.

6. Lateral condyle fracture of a child's humerus: the radiographic features may be subtle

Diagnostic difficulty arises with these fractures which, if neglected, are prone to non-union and subsequent long-term morbidities including pain, instability, cubitus valgus deformity and tardy ulnar nerve palsy.
