Integration psychosozialer Versorgung in die Onkologie Bedarf,Implementierung und Inanspruchnahme am Beispiel des Herforder Modells

Im Rahmen des Herforder Modells wurde 19951997 mit finanzieller Unterstützung der SULO-Stiftung Herford der Aufbau eines umfassenden psychoonkologischen Dienstes am Klinkum Kreis Herford geleistet. Konzeption und Durchführung der Implementierung schlossen von Anbeginn psychosoziale Leistungen für Patienten und Fortbildung für Ärzte und Pflegekräfte ein, um so top down und bottom up-Strategien zu kombinieren. Die Bedarfserhebung zu Projektbeginn bekräftigt aus Patientensicht klare Erwartungen an die Psychoonkologie, generell im Sinne von Hilfe bei der Krankheitsbewältigung, aber auch im Hinblick auf eine enge Kooperation mit der somatischen Behandlungsseite. Unter den konkret gewünschten Maßnahmen spielen vor allem auch Gesprächsangebote in der ambulanten Nachsorge, psychologische Schmerztherapie, Sozialberatung und Gruppenangebote des Entspannungstrainings und des Rehabilitationssports eine wichtige Rolle. Die Patientenversorgungs-Dokumentation weist über einen ca. 24monatigen Zeitraum (bei durchnittlich 2 psychosozialen Fachkräften) 1747 Kontakte mit 516 Patienten auf. Hauptanlässe bzw. -themen der psychoonkologischen Interventionen waren Verarbeitungsdefizite, depressive Reaktionen, Angststörungen und Complianceprobleme. Insgesamt werden damit Bedarf und Inanspruchnahme psychooonkologischer Versorgung differenziert belegt.

In a pilot project in Herford/Germany a psychooncology service has been implemented at a hospital for acute care (supported by the SULO-Stiftung). Conception and implementation strategies included psychosocial care for the patients as well as psychosocial training for doctors and the staff from the early beginning, thus combining the strategies of top down and bottom up. The assessment of patients needs confirms expectations of the patients towards psychooncology, in general with respect to support of coping, but also with respect to a close cooperation with the somatic caregivers. Among the methods offered especially psychosocial aftercare, psychological pain treatment, social counseling as well as groups for relaxation training and rehabilitation sports are rated as important. Documentation data of psychooncological care (24 months experience with 2 psychosocial caregivers on average) show 1747 contacts with 516 cancer patients, dealing with the main issues coping deficits, depressive reactions, anxiety and compliance problems. Overall it could be clearly confirmed, that there is a great need for psychosocial care, and cancer patients make use of it.
