Zur therapeutischen Nutzung von Placeboeffekten im Rahmen einer schmerztherapeutischen Behandlung mit Transkutaner Elektrischer Nervenstimulation

In der vorliegenden Studie wurde versucht, den Placebo-Anteil am Gesamterfolg einer Schmerzbehandlung mit Transkutaner Elektrischer Nervenstimulation (TENS) erkennbar und nutzbar zu machen. Bei 37 ambulanten Schmerzpatienten wurde untersucht, ob der Einsatz eines Oszilloskops während der ersten Einweisungssituation erstens die kurzfristige schmerzlindernde Wirkung von Placebo-TENS steigern kann und zweitens den langfristigen Verlauf der TENS-Therapie günstig beeinflusst. Als Hypothese galt, das Oszilloskop werde durch Sichtbarmachen des unanschaulichen elektrischen Strom suggestiv verstärkend wirken. Die Daten der kurzfristigen Schmerzlinderung legen eine positive Wirkung der zusätzlichen visuellen Wahrnehmung auf die analgetische Wirksamkeit von Placebo-TENS nahe. Auf das reine Placebo-TENS reagierten 27 % der Probanden mit einer Schmerzlinderung, nach zusätzlichem Einsatz des Oszilloskops waren es 53 %, nach Verum-TENS plus Oszilloskop 68 %. Damit ist der Unterschied zu der reinen Placebo-Bedingung bei Einsatz eines Oszilloskopes deutlicher ausgeprägt als die Veränderung zur Verum-Bedingung. In einer Ein-Jahres-Katamnese zeigte sich ein signifikanter Zusammenhang zwischen dem Ausmaß der kurzfristigen und der langfristigen Schmerzreduktion. Der Kliniker wird auf den generellen, richtunggebenden Stellenwert der Ersteinweisung bei TENS-Behandlungen für die Motivation und Mitarbeitsbereitschaft des Patienten hingewiesen. The present study evaluated the therapeutical utilization of placebo effects in pain therapy with transcutaneous electrical stimulation (TENS). A sample of 37 pain patients was studied to analyze the use of an oscilloscope during the first instruction session. The aim of the study was, firstly, to examine whether the use of an oscilloscope can enhance the immediate analgesic effect of placebo-TENS and, secondly, to investigate whether the TENS therapy has a lasting positive influence on pain outcome. It was hypothesized that the oscilloscope would facilitate the suggestive imagination of the patients by visualizing the electricity. The results of transitory pain relief suggest a positive effect of the additional visual perception on the analgesic effects of placebo-TENS. While only 27 % of the participants showed a pain relieving reaction to the placebo-TENS, 53 % of the patients with additional use of an oscilloscope reported pain relief. Pain relief was reported after verum TENS plus oscilloscope by 68 %. A one year follow-up showed a significant association between the extent of the transitory and lasting effects in pain reduction. The clinician will be pointed to the most important implications of the first instruction session by TENS treatment with respect to the motivation and compliance of the patient.
