Psychodiagnostische Untersuchung im Rahmen der Indikationsstellung bei geplanter gastric banding-Operation zur Gewichtsreduktion: Konzept,klinische Erfahrung und erste Evaluationsergebnisse

Die bisherigen Ergebnisse der konservativen Behandlung der Adipositas sind nicht zufrie-denstellend. Höchstens jeder zehnte Versuch der Gewichtsreduktion hat dauerhaften Erfolg. Aus diesem Grund interessieren bzw. entscheiden sich immer mehr Patienten mit der Diag-nose Adipositas permagna (Body-Mass-Index > 40 kg/m^2 ) für eine dauerhafte Gewichtsre-duktion mit Hilfe einer gastric banding-Operation (Einschränkung der Nahrungsaufnahme mittels eines verstellbaren Silikonringes, der um den oberen Teil des Magens gelegt wird). Im vorliegenden Artikel wird das Konzept der psychodiagnostischen Untersuchung im Rahmen der Indikationsstellung bei geplanter gastric banding-Operation vorgestellt. Ziel dieser Untersuchung ist es zum einen, psychisch auffällige Patienten zu identifizieren (Ausschlussdiagnostik). Zum anderen wird eine Eignungsdiagnostik bezüglich der Compliance erwar-tet. Anhand der Stichprobe von 100 Patienten werden die ersten klinischen Erfahrungen mit dem Untersuchungskonzept geschildert sowie die Schwierigkeiten bei der Suche nach den Erfolgsprädiktoren aufgezeigt. Auf der Grundlage von 37 Katamnesen werden die ersten Ergebnisse nach 6 Monaten (signifikante Verringerung des Body-Mass-Index von 47,5 kg/m^2 auf 37,5 kg/m^2, signifikante Verbesserungen der Depressivität, der Selbstakzeptanz sowie positive Veränderungen des Essverhaltens) präsentiert und diskutiert.

The results of conservative obesity therapy obtained so far have been unsatisfactory. Only every tenth attempt to lose weight results in a long-lasting success. Thus, more and more patients with morbid obesity diagnosis (Body-Mass-Index > 40 kg/m^2 ) get interested in or decide to undergo a gastric banding operation in order to lose weight. In gastric banding, an adjustable silicon band, which is applied on the upper part of the stomach, limits the amount of consumed food. This paper presents the concept of psychodiagnostic testing that is part of the assessment of indications/contraindications carried out before a planned operation. The aim of the examination is to both identify patients with psychological disturbances and evaluate their compliance. The first clinical trials regarding the application of this examination method are presented on the basis of 100 patients. The difficulties in finding predictors of success are depicted. The paper presents and discusses the initial results based on the 6 months follow-up of 37 patients (significant decrease of Body-Mass-Index from 47,5 kg/m^2 to 37,5 kg/m^2, decrease of depression, increase of self-acceptance and positive changes in eating behaviour).
