Mandibular reconstruction has been experiencing an amazing evolution. Several different approaches are used to reconstruct this bone and therefore have a fundamental role in the recovery of oral functions.

This review aims to highlight the persistent problems associated with the approaches identified, whether bone grafts or prosthetic devices are used. A brief summary of the historical evolution of the surgical procedures is presented, as well as an insight into possible future pathways.

A literature review was conducted from September to December 2012 using the PubMed database. The keyword used was “mandible reconstruction.” Articles published in the last three years were included as well as the relevant references from those articles and the “historical articles” were referred. This research resulted in a monograph that this article aims to summarize.

Titanium plates, bone grafts, pediculate flaps, free osteomyocutaneous flaps, rapid prototyping, and tissue engineering strategies are some of the identified possibilities. The classical approaches present considerable associated morbidity donor-site-related problems.

Research that results in the development of new prosthetics devices is needed. A new prosthetic approach could minimize the identified problems and offer the patients more predictable, affordable, and comfortable solutions.

This review, while affirming the evolution and the good results found with the actual approaches, emphasizes the negative aspects that still subsist. Thus, it shows that mandible reconstruction is not a closed issue. On the contrary, it remains as a research field where new findings could have a direct positive impact on patients’ life quality. The identification of the persistent problems reveals the characteristics to be considered in a new prosthetic device. This could overcome the current difficulties and result in more comfortable solutions. Medical teams have the responsibility to keep patients informed about the predictable problems related with each elected approach, even understanding that a perfect reconstruction is a secondary goal when compared with maintenance of life.

Flexible flat foot is described as a reduction in the height of the medial longitudinal arch and may occur from abnormal foot pronation. A foot orthosis is thought to modify and control excessive pronation and improve arch height.

To compare the immediate effect of three types of orthoses on foot mobility and the arch height index in subjects with flexible flat feet.

A quasi-experimental study.

The dorsal arch height, midfoot width, foot mobility and arch height index were assessed in 20 participants with flexible flat feet (mean age = 23.2 ± 3 years) for three different foot orthosis conditions: soft, semi-rigid and rigid University of California Biomechanics Laboratory (UCBL).

Maximum midfoot width at 90% with arch mobility in the coronal plane was shown in the semi-rigid orthosis condition. The semi-rigid orthosis resulted in the highest mean foot mobility in 90% of weight bearing, and the rigid orthosis (UCBL) had the lowest mean foot mobility. The soft orthosis resulted in foot mobility between that of the rigid and the semi-rigid orthosis. UCBL orthosis showed the highest arch height index, and the semi-rigid orthosis showed the lowest mean arch height index.

Due to its rigid structure and long medial–lateral walls, the UCBL orthosis appears to limit foot mobility. Therefore, it is necessary to make an orthosis that facilitates foot mobility in the normal range of the foot arch. Future studies should address the dynamic mobility of the foot with using various types of foot orthoses.

Although there are many studies focussed on flat foot and the use of foot orthoses, the mechanism of action is still unclear. This study explored foot mobility and the influence of foot orthoses and showed that a more rigid foot orthosis should be selected based on foot mobility.

The use of a finger prosthesis following finger amputation is a common approach that is linked to many factors.

The aim of this study was to evaluate the effect of a new method of suspension on quality of life, satisfaction, and suspension in these patients.

Quasi-experimental.

A total of 24 patients with finger amputation and 12 healthy subjects as control group participated in this study. Two types of finger prostheses with conventional suction suspension and new method suspension were provided. A force gauge was used to evaluate suspension force along with a World Health Organization Quality of Life-BREF questionnaire instrument. Satisfaction of the prosthesis was also evaluated using a visual analog scale.

There was significant difference in quality of life assessment between the patient group and healthy control group, but there was no statistical difference (p > 0.05) between the two patients groups at baseline and after prosthesis use. Using finger prosthesis improved all domains after prosthesis wearing in patients with finger amputations. There were significant differences (p < 0.05) between the two types of prosthesis in terms of suspension and satisfaction.

Wearing the prosthesis with the new method of suspension had an effective role in terms of providing suspension and an increase in satisfaction in patients with finger amputation.

Patients with finger amputation usually use a prosthesis with a simple suspension technique, yet many of these patients experience atrophy and subsequently loss of appropriate suspension. This paper presents a new method of suspension and showed that using this approach improved satisfaction and suspension in patients with finger amputation.

User feedback about grasping force and hand aperture is very important in object handling with myoelectric forearm prostheses but is lacking in current prostheses. Vibrotactile feedback increases the performance of healthy subjects in virtual grasping tasks, but no extensive validation on potential users has been performed.

Investigate the performance of upper-limb loss subjects in grasping tasks with vibrotactile stimulation, providing hand aperture, and grasping force feedback.

Cross-over trial.

A total of 10 subjects with upper-limb loss performed virtual grasping tasks while perceiving vibrotactile feedback. Hand aperture feedback was provided through an array of coin motors and grasping force feedback through a single miniature stimulator or an array of coin motors. Objects with varying sizes and weights had to be grasped by a virtual hand.

Percentages correctly applied hand apertures and correct grasping force levels were all higher for the vibrotactile feedback condition compared to the no-feedback condition. With visual feedback, the results were always better compared to the vibrotactile feedback condition. Task durations were comparable for all feedback conditions.

Vibrotactile grasping force and hand aperture feedback improves grasping performance of subjects with upper-limb loss. However, it should be investigated whether this is of additional value in daily-life tasks.

This study is a first step toward the implementation of sensory vibrotactile feedback for users of myoelectric forearm prostheses. Grasping force feedback is crucial for optimal object handling, and hand aperture feedback is essential for reduction of required visual attention. Grasping performance with feedback is evaluated for the potential users.

There is a paucity of evidence regarding the design of children’s footwear in relation to musculoskeletal structure.

The aim was to gain consensus regarding where flexibility and support should be given on a school shoe for the 6- to 10-year-olds.

Delphi Questionnaire.

Recruitment of 10 Allied Health Professionals yielded a consensus group. Those recruited had dedicated experience in paediatrics and foot health. Rounds of questions were delivered to investigate the basis of the opinions made by the group on the location of support and flexibility in a school shoe for the age range identified.

Six themes were generated. Four themes gained 100% consensus regarding footwear allowing normal foot movement, footwear allowing normal forefoot function, footwear providing a stable base and the foot being secure in the shoe. Fleiss’ kappa calculations revealed ‘poor agreement’ for the remaining two themes regarding clinical intervention and the purpose of footwear design in the midfoot in both the upper and sole of the shoe.

The qualitative data generated through discussion have highlighted areas where more understanding and research are required, particularly in understanding how, in the long term, shoe design can affect the developing foot.

Children’s footwear advice is often delivered from experience and personal belief. This article questions current understanding and opinion from clinical experts in the field of paediatric footwear and highlights that there is a lack of knowledge and confidence into the effects of children’s footwear. There is a strong requirement for further empirical research to be completed on children’s footwear to allow clinicians to formulate relevant and appropriate footwear advice.

There are many associated devices using impedance method. However, they cannot completely suppress tremor. Other methods using active vibration control have been a challenging problem for industrial and academic researchers. Potential applications of the latter for tremor patients are still not realized.

This article presents a novel method for suppressing human lower arm tremor in essential tremor by using a self-balancing technique.

In this article, a rotating unbalanced mass actuator was used to imitate an arm tremor with the same frequency range obtained from clinical sources. A novel counter-balance device was designed to attach the fictitious arm. The tremor reduction is expecting quantitatively.

A Proportional–Integral–Derivative controller and an algorithm programmed in LabVIEW are proposed to control a rotating unbalanced mass actuator, anti-shaker, in order to counter the tremor. The self-balancing system for the new approach was obtained when both amplitudes and frequencies of anti-shaker and tremor are the same.

The results show that this method can reduce vibration of tremor in the order of 0.001 rad/s².

This method can suppress the vibration and can also auto-tune to counter the tremor in a range of 4–12 Hz bandwidth.

The therapy of arm tremor was associated with using counter-devices to palliate the vibration magnitude. Conventional methods such as physical surgery and drugs cannot completely eliminate tremor. The novel counter-device is an alternative technique with promising application for clinical uses for tremor patients without involving harmful surgery.

This study reviewed prevalence of patients with lower limb amputations with above normal weight profile, with body mass index over 25, in seven disablement services centres managing their amputee rehabilitation in the United Kingdom.

To review two clinical standards of practice in amputee rehabilitation. Ambulant lower limb amputees should have their body weight recorded on an electronic information system, with identification of cohort with body weight >100 kg. Lower limb amputees to be provided with suitable weight-rated prosthesis.

Observational study of clinical practice.

Data were collected from the Clinical Information Management Systems. Inclusion criteria – subjects were ambulant prosthetic users with some prosthetic intervention in the last 5 years and had at least one lower limb amputation.

In 96% of patients, the weight record profile was maintained. In addition, 86% were under 100 kg, which is the most common weight limit of prosthetic componentry. Of 15,204 amputation levels, there were 1830 transfemoral and transtibial sites in users with body weight over 100 kg. In 60 cases, the prosthetic limb build was rated to be below the user body weight.

In 96% of our patients, body weight was documented, and in 97%, the prosthetic limb builds were within stated body weight limits, but this may not be the case in all the other disablement services centres in the United Kingdom. Also, the incidence of obesity in the United Kingdom is a growing problem, and the health issues associated with obesity are further compounded in the amputee population.

Prosthetic componentry has distinct weight limits which must be considered during prescription. As people with amputation approach the limits of specific components, clinicians are faced with the challenge of continued provision in a safe and suitable manner. This article reviews the amputee population and the current national profile to consider trends in provision and the incidence of these challenges.

This study introduces the importance of the aerodynamics to prosthetic limb design for athletes with either a lower-limb or upper-limb amputation.

The study comprises two elements: 1) An initial experiment investigating the stability of outdoor velodrome-based field tests, and 2) An experiment evaluating the application of outdoor velodrome aerodynamic field tests to detect small-scale changes in aerodynamic drag respective of prosthetic limb componentry changes.

An outdoor field-testing method is used to detect small and repeatable changes in the aerodynamic drag of an able-bodied cyclist. These changes were made at levels typical of alterations in prosthetic componentry. The field-based test method of assessment is used at a smaller level of resolution than previously reported.

With a carefully applied protocol, the field test method proved to be statistically stable. The results of the field test experiments demonstrate a noticeable change in overall athlete performance. Aerodynamic refinement of artificial limbs is worthwhile for athletes looking to maximise their competitive performance.

A field-testing method illustrates the importance of the aerodynamic optimisation of prosthetic limb components. The field-testing protocol undertaken in this study gives an accessible and affordable means of doing so by prosthetists and sports engineers.

Using simple and accessible field-testing methods, this exploratory experiment demonstrates how small changes to riders’ equipment, consummate of the scale of a small change in prosthetics componentry, can affect the performance of an athlete. Prosthetists should consider such opportunities for performance enhancement when possible.

At the age of 18 years, jazz guitarist Django Reinhardt (1910–1953) sustained significant burns to his left-hand ring and little fingers; yet, subsequently, he relearned to play and achieved international fame, despite his injuries.

Archive film footage and novel motion analysis software were used to compare movements of Django’s fretting hand with that of six other guitarists of the same genre.

Django employed greater abduction of index and middle fingers (−9.11 ± 6.52° vs −5.78 ± 2.41°; p < 0.001) and more parallel alignment of fingers to the guitar neck (157.7 ± 3.37° vs 150.59 ± 2.67°; p < 0.001) compared to controls.

In response to debilitating hand injury, Django developed quantifiable compensatory adaptation of function of his remaining functional fingers by developing an original playing technique.

Hand function following injury may be optimized by maximizing latent degrees of freedom in remaining digits, rather than through extensive surgical reconstruction or complex prostheses. Further study of adaptation strategies may inform prosthesis design.

Typical upper limb prostheses may limit sports participation; therefore, specialized terminal devices are often needed. The purpose of this study was to evaluate the ability of transradial amputees to play golf using a specialized terminal device.

Club head speed, X-factor, and elbow motion of two individuals with transradial amputations using an Eagle Golf terminal device were compared to a non-amputee during a golf swing. Measurements were collected pre/post training with various stances and grips.

Both prosthesis users preferred a right-handed stance initially; however, after training, one preferred a left-handed stance. The amputees had slower club head speeds and a lower X-factor compared to the non-amputee golfer, but increased their individual elbow motion on the prosthetic side after training.

Amputees enjoyed using the device, and it may provide kinematic benefits indicated by the increase in elbow flexion on the prosthetic side.

The transradial amputees were able to swing a golf club with sufficient repetition, form, and velocity to play golf recreationally. Increased elbow flexion on the prosthetic side suggests a potential benefit from using the Eagle Golf terminal device. Participating in recreational sports can increase amputees’ health and quality of life.

Microprocessor-controlled prosthetic knees have gained increasing popularity over the last decade. Research supports their provision to address specific problems or to achieve certain rehabilitation goals. However, there are yet no agreed protocols or prescribing criteria to assist clinicians in the identification and appropriate selection of suitable users. The aim is to reach professionals’ agreement on specific prescribing guidelines for microprocessor-controlled prosthetic knees.

The study involved multidisciplinary teams from the Inter Regional Prosthetic Audit Group, representing nine Prosthetic Rehabilitation Centres in the South East England region. We used the Delphi technique with a total of three rounds to reach professionals’ agreement.

The prescribing guidelines were agreed and will be reviewed and updated depending on new research evidence and technical advances.

This project is highly useful for professionals in a clinic setting to aid in appropriate patient selection and to justify the cost of prescribing microprocessor-controlled prosthetic knees.

Several studies have shown that feedback in upper-leg prostheses is possible, but slow or difficult to interpret. In this study, electrotactile and auditive error-based feedback, only giving feedback when an undesired event occurs, were tested for its use in upper-leg prosthesis when provided during a perturbation.

A total of nine healthy subjects walked on a prosthetic simulator which was disturbed at the end of the swing phase. They received either no feedback, electrotactile feedback, or auditive feedback at the time of the perturbation.

The reaction time of the subjects only improved by 40 ms when using auditory feedback, compared to the no-feedback condition. No changes in reaction time were found in the electrotactile feedback condition. Considering perturbation detection was not taken into account in this study, this improvement is not enough for practical applications in upper-leg prosthesis.

Many transfemoral amputees are insecure about their prosthesis, are afraid of falling, or actually fall. Providing feedback specifically during a perturbation may prevent them from falling, or at least give them a chance to react.
