We congratulate Sim and colleagues on their successful completion of a completely intracorporeal laparoscopic ileal interposition. Although further study is needed, it is likely that patients may benefit from avoiding a larger incision to perform ileal isolation as we have also described robotically. ¹

We agree that undocking and redocking of the robot could potentially increase operative time. Nevertheless, one less trocar was needed with the robotic technique and the hospital stay was 2 days less than with the laparoscopic technique. Furthermore, the ergonomics with robotics are preferable, especially with a complex reconstructive procedure. The latest version of the da Vinci robot is specifically designed to facilitate procedures performed in multiple abdominal quadrants and will likely make procedures such as these even more efficient.
