Abstract

On Sunday, 11 December 2005, the largest fire and explosion in the United Kingdom in peacetime occurred and made headlines in the National and International news media. The Buncefield Explosion shook the entire petroleum industry as well as the safety regulators.
Investigations uncovered a wide range of contributing factors to the incident and the ultimate consequences of the explosion. But are the ‘lessons to be learned’ only confined to the storage of fuels or are there wider issues of interest and application within management, for instance?
Colin Howard, CEng, HonFInstMC, is in an excellent position to explore this, as he was an expert witness to the court for instrumentation and control aspects of the criminal case. Colin is a past President of the InstMC and a consultant specialising in process safety and the use of instrumentation and control for functional safety.
Those with a responsibility for the safe storage or processing of highly flammable or hazardous materials should certainly attend this excellent lecture.
